Partnerships for Care

North Simcoe Muskoka Specialized Geriatric Services has worked with two of our area Ontario Health Team (OHT) partners over the past year to help ensure our local health care providers are knowledgeable and aware of local resources. Since COVID, there have been many changes in team members and resources across the region. Providers connect frequently through virtual platforms like Teams or Zoom.  As such, we’ve lost some of the personal contact that we had in this region that made our care so special. Relationships and trust are integral to providing quality, collaborative care to our older adults and their care partners. 

In June 2024 - Seniors’ Month - South Georgian Bay OHT’s Supporting Seniors Action Team hosted an event at Blue Mountain for area providers. This event, done in collaboration with NSM SGS, focused on showcasing area resources and services and building relationships among providers. Through a variety of networking activities and a guided case study, partners gained a better understanding of the people and programs in their region. The buzz in the room was amazing and business cards were passed around readily!!  

In October, the Barrie Area OHT Frail Seniors’ Working Group hosted a similar event for frontline providers. The afternoon was well-attended and allowed 40 front-line providers to come together to build relationships, showcase their services, participate in generative group discussion, and engage in case scenarios.  In December, the Barrie Area OHT hosted a second event with leaders where the focus was on defining opportunities and key projects for the coming year to support older adults and care partners in the region.

NSM SGS is so thankful to our partners for the collaboration opportunities that have occurred and hope for more such activities going forward to ensure our older adults receive the best care possible, close to home.
