GeriMedRisk

North Simcoe Muskoka Specialized Geriatric Services is the first satellite partner site of the provincial GeriMedRisk program. GeriMedRisk is a non-profit virtual clinical service that connects Ontario physicians, nurse practitioners and pharmacists with geriatric specialists to optimize medications, physical and mental health conditions in older adults.  Our interdisciplinary team of specialists including geriatric psychiatry, geriatric medicine, clinical pharmacology and NSM SGS pharmacist, work collaboratively to provide support in medication management including:

• Optimizing medications or developing a de-prescribing plan.
• Identifying drug interactions and adverse drug events.
• Providing information about drug-specific adverse drug events.
• Troubleshooting medication management in complex medical and mental health conditions. 

Upon receipt of all required information, a comprehensive patient-specific consultation note and relevant educational materials are typically sent back within five (5) business days.

Access to the provincial GeriMedRisk program is through SGS Central Intake. However, GeriMedRisk can also be accessed through the OTNhub eConsult service (GeriMedRisk/ Geriatric Clinical Pharmacology or Geriatric Medication Provincial Group specialty groups) or by calling GeriMedRisk at 1-855-261-0508.
