As part of the NSM SGS Mental Health Community team, the Community Support Worker (CSW), works collaboratively within an interdisciplinary team. As part of the interdisciplinary team, this individual will support older adults and their care partners (formal/informal) living with cognitive impairment and responsive behaviours in community-based settings across the NSM region. The cognitive impairment may be related to dementia, mental illness, substance use or neurodegenerative disease processes. The role provides expertise and guidance through observation, person centred strategy development and implementation and coaching and mentoring for those experiencing behavioural expressions. The CSW position also assists with the implementation of care plan interventions that are developed collaboratively within the team environment. This is a regional program serving all of North Simcoe-Muskoka. Each CSW is assigned a primary region however, may be required to work in other NSM regions based on client and program needs. This posted position is for the North Simcoe (Midland/Penetanguishene) and South Georgian Bay (Collingwood/Wasaga Beach) regions. Travel between these regions is required.
